Tested it with audio sync off and fps limit on in openGL with vsync on and off, honestly I don't think vsync does anything atm.

So that build works pretty much perfectly now, in both window and FS modes no more micro strutter after 1/2sec, it's 100% smooth, but unfortunate after some time, almost 5min, it starts to strutter constantly then sometimes it goes back to the 100% smooth or it keeps the messed up frame pace, now when it doesn't automaticaly goes back I noticed that if I enable audio sync then disable the audio sync, after it disable fps limit and then enable it gets back to the 100% smooth, however again after some time (5m or so) it is stats to mess up, maybe it's related with how many screen I have passed in the game? Sometimes I get in one room that strutters and then the other is not, but when it happens certainly it will start to strutter very soon.
